Decision Aid for Goals of Care

Description

The Decision Aid for Goals of Care tool can help nursing home clinicians share information about goals of care and treatment options with families of people with late-stage dementia, and then incorporate decisions into the plan of care.

This decision aid is first shared with family members in order to prepare and educate them about health care decisions for people with advanced dementia.

Generously funded by the National Institute on Aging.
