The Decision Aid for Goals of Care is a decision aid and care planning tool for nursing home clinicians.

It helps facilitate conversations with families of people with late-stage dementia, sharing information about goals of care and treatment options. Clinicians can then incorporate these decisions into the individual's plan of care.
